TRANSFER OF RECORDS -- If you are entering our practice, please make arrangements with your former physician for the transfer of all essential medical information to our office. If you are leaving our practice, we extend the same courtesy to your new physician. IF YOU REQUIRE A COPY OF RECORDS FOR YOURSELF, THERE IS AN ADMINISTRATIVE FEE OF $1.00 PER PAGE FOR THE FIRST 25 PAGES AND THEN $.50 PER PAGE THEREAFTER. THIS IS IN ACCORDANCE WITH STANDARD PUBLISHED POLICIES OF THE FMA.

APPOINTMENTS --Phone calls for appointments hsould be made early in the day. If your child becomes ill during the day, DO NOT wait until after supper to call. Evening calls should be for urgent problems or emergencies, and not for matters that could have been handled during office hours. Please call 24 hours in advance to cancel an appointment so that we can put someone else in your place. We reserve the right to charge for non-canceled, missed appointments.

INSURANCE PLAN/REFERRALS -- If your insurance company requires
approval for visits to specialists, ER's or special equipment, you must
contact our office prior to these visits. We do not give retroactive
referrals. You are expected to know the limitations of your insurance
company.
PRACTICE
INFORMATION
PHONE CALLS --We wish to thank you for the consideration you
have shown us after hours. Physicians have families and friends
that they enjoy too. If something of a non-urgent nature arises,
please call during regular office hours. If it is an emergency,
please call 911 IMMEADIATELY. Please do not hesitate to call AFTER
HOURS for urgent, immediate care. We reserve the right to charge
for excessive as well as prolonged telephone calls.
EMERGENCIES --In any suspected EMERGENCY situation, call 911. In
an URGENT situation call the office number. If no one is in the
office, page Donna at 456-6156. If there is no answer in a half an
hour, you may call back to the office and wait for the answering
service to come on and they will locate the physician on call for you.
SICK VISITS -- If you are calling to establish whether a sick child
should be seen, please take the child's temperature and report this
along with their pertinent information such as weight. To speak to a
nurse when calling the office, ask for extension 18. Patients will be
seen on the day of the mother's request if she feels it is an
emergency. If there is difficulty in this request, please ask to
speak to a physician.
ANSWERING MACHINE --the answering machine will give you a
variety of information about the practice including mailing address
and directions for the Merritt Island Office. For after hours, it will
give you the pager number of the on call person.
PRESCRIPTIONS -- Prescriptions will only be filled during office
hours. Please do not call for a refill unless during the day. You
may leave a message for a refill on extension 25. There is a 24 to
36 hour turn around time on most refills. You may also ask for a
refill from your pharmacist who can email us on the EMPOWERX
system or FAX for a faster service.
